APPLICATION FOR ENROLMENT FORM – INTERNATIONAL STUDENTS

Thank you for your interest in seeking enrolment into St Peter Institute. This application for enrolment form must be completed in English. If you require any assistance in completing this form, please contact us by phone or email. You can send this form to us by post or email. Please provide a copy of your passport, your visa (if relevant), your High School Certificate, proof of English language proficiency as specified in the entry requirements and any other requested documents referred to in the course brochure.

Why we collect your personal information

As a registered training organisation (RTO), we collect your personal information so we can process and manage your enrolment in a vocational education and training (VET) course with us. If you do not provide this information, we will be unable to process your enrolment.

How we use your personal information

We use your personal information to enable us to deliver VET courses to you, and otherwise, as needed, to comply with our obligations as an RTO.

How we disclose your personal information

We are required by law (under the National Vocational Education and Training Regulator Act 2011 (Cth) (NVETR Act)) to disclose the personal information we collect about you to the National VET Data Collection kept by the National Centre for Vocational Education Research Ltd (NCVER). The NCVER is responsible for collecting, managing, analysing and communicating research and statistics about the Australian VET sector.

We are also authorised by law (under the NVETR Act) to disclose your personal information to the relevant state or territory training authority.

How the NCVER and other bodies handle your personal information

The NCVER will collect, hold, use and disclose your personal information in accordance with the law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act. Your personal information may be used and disclosed by NCVER for purposes that include populating authenticated VET transcripts; administration of VET; facilitation of statistics and research relating to education, including surveys and data linkage; and understanding the VET market.

DEWR is authorised by law to collect, use and disclose your personal information. For more information, please refer to the DEWR VET Privacy Notice at DEWR Privacy Notice.

Surveys

You may receive a student survey conducted by a government department or NCVER. You may opt out at the time of being contacted.

Unique Student Identifier (USI)

You grant the Institute permission to search for your USI number should the need arise.

Language, Literacy, Numeracy (LLN) and Digital Skills

Once enrolled, St Peter Institute will review your Language, Literacy, Numeracy and Digital skills to identify if you need any support and determine the most suitable support for your learning needs.

Disability Supplement

Introduction

The purpose of the Disability Supplement is to provide additional information to assist with answering the disability question. If you indicated the presence of a disability, impairment or long-term condition, please select the relevant area(s) below.

Disability in this context does not include short-term conditions such as a fractured leg, influenza, or corrected physical conditions such as impaired vision managed by glasses or lenses.

  • 11 — Hearing/deaf
    Hearing impairment refers to a person with mild to profound hearing loss who communicates orally and may use amplification. A person who is deaf has severe or profound hearing loss from birth or early life and mainly relies on visual communication such as lip reading or sign language.
  • 12 — Physical
    A physical disability affects mobility or dexterity and may include loss of body function or part. It may result from birth, illness, or injury (e.g. arthritis, cerebral palsy, multiple sclerosis, paraplegia).
  • 13 — Intellectual
    Refers to low intellectual functioning and difficulties in adaptive behaviour before age 18, possibly due to infection, trauma, or illness.
  • 14 — Learning
    Refers to difficulties in learning skills such as reading, writing, listening, speaking, or mathematics due to neurological factors.
  • 15 — Mental illness
    Refers to psychological conditions causing distress and affecting normal functioning.
  • 16 — Acquired brain impairment
    Brain injury affecting cognitive, physical, or emotional functioning due to trauma, illness, or other causes.
  • 17 — Vision
    Partial or total loss of sight, from mild impairment to blindness.
  • 18 — Medical condition
    A condition that may be temporary or permanent (e.g. asthma, diabetes, cancer), affecting health and functioning.
  • 19 — Other
    Any disability not covered above, including autism spectrum disorders.
